Bleeding disorders and anticoagulant therapy in dentistry
General Medicine and General Surgery (for BDS) · Dental (BDS)
Study notes
A warfarin patient needs an extraction with INR 2.8. Should warfarin stop? Usually no - for therapeutic INR, continue with local hemostatic measures (pressure, sutures, tranexamic acid); stopping risks thrombosis worse than the bleeding. Students learn the liaison rule: the physician decides drug changes, the dentist provides local hemostasis - because the clot that prevents stroke outranks the socket that bleeds a little longer.
