A 25-year-old patient arrives with sharp, throbbing pain in the lower right molar that worsens at night. Clinical exam: Deep cavity reaching the pulp. Percussion test: Positive (pain when tapping the tooth). Diagnosis: Irreversible Pulpitis. Treatment: 1. Administer 2% Lidocaine with 1:100,000 epinephrine for anesthesia. 2. Place a rubber dam. 3. Access the pulp chamber with a high-speed diamond bur. 4. Use K-files to reach the root tip (verified by apex locator). 5. Irrigate with 5.25% NaOCl. 6. Dry with paper points and fill with Gutta-percha and sealer (AH Plus). Outcome: Pain resolves immediately. Follow-up: X-ray after 6 months confirms bone healing around the root tip.
