A 25-year-old patient comes in with a deep cavity in the lower right second molar. Diagnosis: Irreversible pulpitis. Treatment steps: 1. Apply topical anesthetic gel (15% lignocaine) to the injection site. 2. Load a 1.8ml cartridge of 4% Articaine with 1:100,000 Adrenaline into an aspirating syringe. 3. Perform an Inferior Alveolar Nerve Block by inserting the needle to the pterygomandibular space. 4. Aspirate to ensure no blood return. 5. Inject 1.5ml slowly over 60 seconds. 6. Wait 5-10 minutes for lip numbness. The patient feels no pain during the drilling; we follow up by checking the site for hematoma (bruising) after 24 hours.
