A child post-trauma can open only 5 mm and X-ray shows condylar fusion. Why is surgery plus physiotherapy the answer? Ankylosis - the joint has fused into bone; no splint or medicine can un-fuse it. Gap arthroplasty removes the bony block, but without aggressive post-operative opening exercises, it re-fuses. Students learn ankylosis as surgery-plus-rehabilitation: the operation opens the mouth, physiotherapy keeps it open.
