A cough of 3 weeks with weight loss — sputum GeneXpert detects TB, rifampicin-sensitive. GeneXpert (CBNAAT) detects TB DNA in 2 hours plus rifampicin resistance — the test that transformed TB programs; ZN smear is faster but misses half. Treatment: 2HRZE/4HR for 6 months under DOTS — every dose observed. Contacts screened, children given prophylaxis. The cough that outlasts antibiotics is TB until proven otherwise — test, treat, complete.
