Chest pain at midnight; troponin at 6 AM is strongly positive. This confirms myocardial infarction — troponin I/T leaks only from dying heart muscle, rising 3–6 hours after onset, peaking at 24 hours, lingering 7–10 days (so it catches late presenters too). The ECG localizes, troponin confirms — together they trigger angioplasty. A negative troponin at 2 hours doesn't exclude (too early — repeat at 6); a positive one rarely lies. Timing is the test's grammar.
