A post-surgery patient has a swollen painful calf; days later sudden breathlessness. The calf is deep vein thrombosis — Virchow's triad after surgery (stasis from immobility, endothelial injury, hypercoagulability). The breathlessness is pulmonary embolism — the clot broke off, traveled through the heart, and lodged in pulmonary arteries. Prevention (early mobilization, heparin) beats treatment; sudden dyspnea + pleuritic pain + tachycardia after DVT is PE until proven otherwise. One clot, two diseases — connected by travel.
