Painless bright-red bleeding on defecation — hemorrhoids; fiber and banding treat. Hemorrhoids (painless bleeding, prolapse — constipation: fiber, fluids; banding/sclerotherapy; hemorrhoidectomy for prolapsing): painless is the clue. Fissure (painful defecation — posterior midline tear: GTN ointment, warm baths; lateral internal sphincterotomy chronic). Fistula-in-ano (perianal discharge — Goodsall's rule predicts track; fistulotomy). Colorectal cancer (altered bowels, bleeding, weight loss — left: obstruction; right: anemia: colonoscopy + biopsy; surgery — colectomy + chemo; screening >45 — FOBT/colonoscopy). Painless bleeding is hemorrhoids — painful is fissure.
