A swelling under the jaw appears at every meal — submandibular stone; sialogram confirms. Sialolithiasis (meal-time swelling + pain — saliva blocked behind the stone; submandibular — Wharton's duct, alkaline mucous saliva): small stones milk out, large need surgery. Mumps (parotid — bilateral, viral, orchitis risk). Tumors — pleomorphic adenoma (parotid benign commonest — slow, facial nerve spared in superficial parotidectomy), Warthin's (smokers), malignant (mucoepidermoid commonest; adenoid cystic — perineural, facial palsy). The meal-time pattern is the diagnosis — saliva against a dam.
