A 2-year-old has swollen feet, thin hair, and a distended belly. This is kwashiorkor — protein deficiency with adequate-ish calories: edema (low albumin), hair changes, fatty liver (potbelly), irritability. Contrast marasmus — total energy starvation: matchstick limbs, old-man face, no edema. Treatment is careful: refeed slowly (refeeding syndrome kills — phosphate crashes), treat infections first, then protein. The belly is not fat but liver and fluid — hunger's deceptive face.
