Sudden painless blindness in one eye — CRAO; cherry-red spot confirms; minutes matter. Central retinal artery occlusion (embolus — carotid/heart: sudden painless monocular blindness, pale retina + cherry-red spot at fovea): the eye's stroke — treat like brain stroke (ocular massage, anterior chamber paracentesis, lower IOP, carbogen — all within hours; benefit limited, so prevention rules). CRVO (vein — blood-and-thunder hemorrhages; hypertension/diabetes: anti-VEGF, pan-retinal laser for ischemia). Amaurosis fugax (transient curtain — carotid TIA: urgent carotid workup). The painless sudden blackness is vascular — time is retina.
