A farmer collapses after spraying — salivating, pinpoint pupils, twitching. Organophosphate poisoning (cholinesterase inhibited — acetylcholine flood): SLUDGE (salivation, lacrimation, urination, defecation, GI, emesis) + muscle twitching/fasciculations + miosis + bradycardia + bronchorrhea. Atropine (dries secretions — titrate to clear lungs) + pralidoxime (reactivates enzyme before aging — within hours) + ventilation + decontamination. Intermediate syndrome (neck weakness days later) and delayed neuropathy follow. The field's smell is the warning — PPE prevents.
