A family found unconscious near a coal brazier — cherry-red skin; carried to fresh air. Carbon monoxide (odorless — faulty heaters/braziers) binds hemoglobin 250× tighter than oxygen — cherry-red skin/blood, headache → coma; 100% oxygen (hyperbaric if severe/pregnant) displaces it; co-oximetry diagnoses (pulse oximetry lies — reads falsely normal). Delayed neuropsychiatric sequelae weeks later. The silent killer is truly silent — detectors, ventilation, and suspicion save.
