Polytrauma bleeding — TXA, blood 1:1:1; FAST positive; OT; survives. Trauma resuscitation: CABC (catastrophic — tourniquet, pressure; airway; breathing — decompress; circulation — 2 large IVs), hemorrhage (direct pressure — tourniquet — junctional; pelvic binder — unstable; TXA — <3h — 1g; blood — 1:1:1 — massive protocol; permissive — SBP 80–90 — except TBI), damage control (surgery — pack, staple — ICU — correct — coagulopathy, acidosis, hypothermia — return: lethal triad), FAST — positive unstable — OT. Stop the bleed — everything else waits.
