Colicky pain, USG shows gallstones — cholecystectomy planned. Abdominal imaging: obstruction (erect X-ray — air-fluid levels — dilated loops: NGT, fluids; CT — transition — strangulation — surgery), gallstones (USG — hyperechoic — shadowing — mobile: cholecystectomy — symptomatic; MRCP — CBD), renal stones (CT KUB — non-contrast — gold: <5mm — pass; 5–10 — ESWL; >10 — PCNL; ureteric — tamsulosin), appendicitis (USG — non-compressible — >6mm; CT — appendicolith: appendectomy), perforation (erect — free air — surgery). The mobile shadow is gallstone — USG diagnoses.
