A hot swollen knee with fever — septic arthritis; aspirated, washed out, antibiotics. Septic arthritis (bacterial — Staph — hot, swollen, single joint — fever: aspirate — pus — WBC high: emergency — IV antibiotics + arthroscopic washout — cartilage dies hours): OA (degenerative — elderly — knee/hip — pain, stiffness <30 min, crepitus: X-ray — joint space loss, osteophytes; treat — weight loss, physiotherapy, paracetamol/NSAIDs; end-stage — joint replacement — TKR/THR), RA (autoimmune — symmetric small — morning stiffness >1h — anti-CCP: DMARDs — methotrexate early). The hot joint with fever is septic — aspirate immediately.
