A heart failure patient on digoxin develops yellow vision and arrhythmia. This is digoxin toxicity — narrow therapeutic index worsened by low potassium (diuretics), kidney failure, or interacting drugs (amiodarone). Signs: anorexia, nausea, yellow-green vision (xanthopsia), any arrhythmia. Digoxin-specific antibody (Fab fragments) reverses severe toxicity. The lesson: monitor levels, potassium, and kidneys — digoxin helps failing hearts but punishes inattention.
