A 6-year-old with sickle cell doubles over with severe bone pain. This is a vaso-occlusive crisis — deoxygenated HbS (glutamate→valine at position 6) polymerizes, red cells sickle and jam microvessels: bone infarcts scream with pain. Triggers: dehydration, infection, cold, hypoxia. Treatment: fluids, oxygen, analgesia (morphine), and hydroxyurea long-term (raises fetal Hb, fewer crises). The single amino acid swap explains everything — molecular medicine's founding story.
