A newborn — BCG, OPV-0, Hepatitis B at birth; card marked for 6-week visit. National Immunization Schedule (birth — BCG, OPV-0, HepB; 6/10/14 weeks — OPV + Pentavalent (DPT+HepB+Hib) + Rotavirus + PCV; 9 months — MR-1, Vitamin A; 16–24 months — MR-2, DPT booster, OPV booster; 5–6y — DPT-2; 10y/16y — Td): cold chain (2–8°C — ILR, cold boxes; freeze-sensitive — DPT, HepB — never freeze; heat-sensitive — OPV, BCG — shake test): adverse events (AEFI — report), catch-up (complete missed — no restart). The card is the record — every dose documented.
