Pressing a swelling near the inner eye releases pus — dacryocystitis; DCR surgery cures. Dacryocystitis (nasolacrimal duct block → sac infection: tender swelling below medial canthus, pus on pressure, epiphora): acute — antibiotics + warm compress; chronic/recurrent — DCR (dacryocystorhinostomy — new drainage into nose). Congenital NLD obstruction (infants — watering from birth: Crigler massage; probing at 1 year if persistent). Epiphora (watering: obstruction — sac/duct; vs hypersecretion — irritation). The pus on pressure is the diagnosis — the sac is infected behind a blocked drain.
