A fever returns every 48 hours with chills — vivax malaria on smear; primaquine is added. Vivax's dormant liver hypnozoites relapse for months — chloroquine clears blood stages, primaquine 14 days kills the liver sleepers; G6PD testing first (hemolysis risk). Falciparum (malignant — cerebral malaria, blackwater fever) needs ACT, severe needs IV artesunate. The periodicity (tertian/quartan) is the clinical poetry — the smear writes the prescription.
