A traveler returns with bloody mucoid stools — amoebic dysentery; metronidazole is given. Entamoeba histolytica (fecal-oral, contaminated water) invades the colon — flask-shaped ulcers, bloody mucoid dysentery; trophozoites may reach the liver (anchovy-sauce abscess, right upper quadrant pain, fever). Metronidazole kills tissue forms; diloxanide clears luminal cysts — both needed or relapse follows. Stool microscopy (cysts/trophozoites) diagnoses. The traveler's dysentery is water's warning — sanitize the source.
