A child wakes with puffy eyes and cola-colored urine two weeks after a sore throat. This is post-streptococcal glomerulonephritis — antibodies against strep deposit in glomeruli (immune complexes), inflaming filters 1–3 weeks after pharyngitis: hematuria (cola urine), periorbital edema, hypertension. It usually resolves with supportive care — the contrast with nephrotic syndrome (massive proteinuria, no hematuria/hypertension) is the bedside split. The throat-to-kidney delay is immune complexes' travel time.
