A 3-year-old's upper front teeth are decayed to stumps from night milk bottles. Why is diet counselling the core treatment? ECC is feeding-driven - milk pooling on teeth all night feeds bacteria; restorations without stopping the bottle fail as new lesions erupt. Students learn ECC's truth: the bottle, not the bacteria, is the disease - treat the feeding pattern first (wean night bottles, wipe teeth), then restore - because dentistry can't outfill a habit.
