An implant with bleeding and 4 mm bone loss appears in a former periodontitis patient who smokes. What went wrong? The classic risk triad - susceptible host, smoking, and plaque - recreated periodontitis around the implant, where it progresses faster without a periodontal ligament's defence. Students learn implant truth: the same biology that destroyed teeth attacks implants - so periodontal patients need stabilized disease and smoking cessation before implants, not after.
