A deep 3-walled vertical defect surrounds one tooth. Why attempt regeneration instead of just cleaning? The defect's three bony walls can contain a graft and membrane, and biologics (enamel matrix proteins) can coax new cementum, ligament and bone - actually rebuilding attachment rather than just a long junctional epithelium repair. Students learn the defect-anatomy rule: contained defects regenerate predictably; the fewer the walls, the poorer the odds - so case selection is the therapy.
