A breech at term — ECV attempted; failed; elective C-section done. Malpresentations (breech — frank/complete/footling: ECV — external cephalic version 37w if eligible; vaginal breech — selected; transverse — ECV or C-section — never vaginal; face/brow — mentum anterior — vaginal; persistent OP — rotate): obstructed labor (no descent despite contractions — Bandl's ring, caput — C-section urgently — rupture risk). C-section (indications — fetal distress, CPD, placenta previa, abruption, previous 2 LSCS, transverse: elective vs emergency; VBAC — one previous LSCS — trial if favorable pelvis). Shoulder dystocia (turtle sign — McRoberts + suprapubic pressure — deliver posterior arm). The transverse lie is C-section — always.
