A pregnant woman — first visit 10 weeks; registered; iron, TT, ultrasound planned. Antenatal care (register <12 weeks — MCP card: visits — ≥4 (8 WHO): history, vitals, weight, BP, edema, fundal height, fetal heart; investigations — Hb, blood group, HIV/syphilis/HBV, urine, OGTT, ultrasound — dating, anomaly 18–20w, growth; supplements — IFA 6 months, calcium; TT/Td — 2 doses; counsel — danger signs — bleeding, headache, reduced movements, leaking): high-risk (teens, >35, short stature, twins, hypertension, diabetes, anemia, previous C-section/stillbirth, Rh-negative: extra visits, specialist). Preconception (folic acid — NTD prevention; optimize — diabetes, rubella vaccine). The MCP card is the record — every visit documented.
